On June **, ****, My cardiologists (Dr. Molly Rutherford) changed my cholesterol medication to Brand Name Zocor ** MG for a ** day supply. She sent it to my local CVS at Hollins Virginia. However, my CVS pharmacy is out of stock and will not be able to get this medication. The pharmacist at CVS Hollins checked other CVS stores in the Roanoke, Va. area and could not find any CVS that had a ** day supply. He suggested that I change the prescription to Caremark mail order for a ** day supply. He said the only thing I had to do to get the Brand name Zocor was to have Caremark call the CVS Hollins VA store and have it changed to mail order. I now have * pills left. I am requesting that you call CVS Hollins at * (***) ***-**** and have my prescription changed to mail order from Caremark. The prescription my Dr. changed me to is BRAND ZOCOR ** MG for ** days supply. I am a high option member and my member ID* is N********. This is NOT a request from me, but my Dr. says the Brand name is medically necessary. I received one prescription from CVS Hollins on *******. Would you please call them and change my prescription to mail order at the High option member price. Again the CVS * is * (***) ***-****? Thank you very much.
